NEW STUDENTS (LOCAL)
- Application Form
- Two (2) Recent '1' ID Pictures
- Photocopy of record card (if any)
- Photocopy of Birth Certificate (NSO/PSA) copy
- Photocopy of Baptismal Certificate (if any)
- Original form 137 Transcript Of Record (TOR) if any
- Entrance Exam Fee (PHP 500.00)
FOREIGN STUDENTS
- Authenticated Form 137/ Transcript Of Record (TOR) from previous school
- Photocopy of Passport / ACR or ICARD for SSP
- 2 (two) 2x2 Picture with white background
